Player Supplemental Insurance

A portion of the fees you pay each session to AASA is for supplemental insurance provided through
the United States Adult Soccer Association (USASA). Each year, registered players sign a waiver
acknowledging the risks associated with playing soccer. A copy of the waiver can be found below.

The insurance can be applied to expenses arising from an injury incurred during an AASA league game,
including: ambulance expenses, hospital visits, X-ray expenses, and other related expenses. For a full

If you are injured while playing an AASA league game, complete section A of the Claim form below,
review the document and sign page two. Then submit the form along with receipts and Explanation of
Benefits (EOB) summary from your primary insurance provider to an AASA board member. If you don’t have
a primary insurance provider a claim can still be submitted. More detailed instructions for filing a
claim can be found below.
